What happens if you join

Four steps. About a minute per reading.

Your provider may recommend one or two readings a day based on your care plan.

What you use at home

Your provider picks only what you need.

Most patients use one device. For this primary care program, the most common equipment is a connected blood pressure monitor or weight scale.

VITASIGNS connected upper-arm blood pressure monitor and cuff
Most common

Blood pressure monitor

Checks your blood pressure and pulse.

You: Wrap it on your upper arm and press one button.
VITASIGNS connected digital weight scale
For heart failure

Weight scale

A few extra pounds over a few days can be an early sign of fluid building up.

You: Step on it at the time your care team recommends. That is it.

Glucose meter

Checks your blood sugar.

You: Take a reading at the times your care team asks for.
FDA-cleared medical devicesNot consumer fitness gadgets.
Supplied, not sold to youIf you stop, you send the device back.
Protected under HIPAANever sold and never used for advertising.
No contractYou may ask to stop at any time.

How it works

Three simple steps. About a minute per reading.

The program is designed for patients who do not want another complicated piece of technology.

1

Your Paloma provider decides if it fits

A Paloma clinician reviews your condition, explains the program, and confirms that you want to participate.

You do not enroll on your own
2

Your equipment arrives ready to use

A VITASIGNS care team member helps you get started. There is no app to download, password to remember, or number to type.

A family member can join the setup call
3

Take a reading and stay connected

Your reading sends automatically. The VITASIGNS clinical team reviews trends and contacts you when follow-up is needed.

Usually one or two readings a day, based on your care plan

Why it can help

A clearer picture between visits.

The value comes from home readings plus regular care-team follow-up, not from the device alone.

See a trend, not a snapshot

Home readings can give your care team more context than a single number taken during an office visit.

Stay connected to a real care team

The VITASIGNS clinical team reviews trends, answers questions, and coordinates with Paloma when clinical follow-up is needed.

Understand changes sooner

Home readings may help your Paloma provider evaluate how a medication or care-plan change is working between visits.

Help reduce the risk of serious complications

Staying on top of blood pressure and heart-failure trends can support earlier follow-up when your care team identifies a concern.

Why staying on top of blood pressure matters

The American Heart Association notes that uncontrolled high blood pressure can contribute to heart attack, stroke, heart failure, and other serious health problems. Monitoring does not guarantee that an emergency or complication will be prevented.

Individual results vary. This program does not guarantee fewer emergencies, hospital visits, complications, or any particular clinical outcome.

Clear cost expectations

Most plans cover eligible patients

Know your expected cost, if any, before you decide.

Coverage and patient costs vary by insurance plan. VITASIGNS will help review your benefits and explain any expected deductible, copay, or coinsurance before enrollment. Depending on your insurance coverage, you may or may not have an out-of-pocket cost.

Request a coverage review

A benefit review is not a guarantee of coverage or payment. Your insurance plan makes the final coverage determination.

Is this right for me?

Often a good fit for current Paloma patients who:

  • Have high blood pressure that is not fully controlled
  • Are living with heart failure
  • Recently left the hospital or changed medication
  • Manage heart disease along with other chronic conditions

Common questions

Questions patients ask.

Do I need a smartphone, computer, or Wi-Fi?

No. The device has its own cellular connection built in. There is nothing to download, no password, and nothing to plug into your internet.

How long does it take each day?

Each reading takes about a minute. Your provider may recommend one or two readings a day based on your care plan.

Who reviews my readings?

The VITASIGNS clinical team reviews your vitals and escalates concerns when needed. Your provider at Paloma remains in charge of your treatment.

What if I miss a day?

A missed day is not an emergency. If readings stop coming in, someone may contact you to make sure the device is working and see whether you need help.

Do I get to keep the device?

No. It is supplied as part of the program, not given or sold to you. If you stop, you return it in a prepaid box.

What if I want to stop?

You may ask to stop at any time. Your team will explain the closeout and device-return process. There is no penalty, and your regular care at Paloma is unaffected.

Does this replace my office visits?

No. You still see your Paloma provider. This program supports the stretches between appointments.

What could it cost?

Most plans cover these services for eligible patients, but benefits vary. VITASIGNS will help review coverage and explain any expected deductible, copay, or coinsurance before enrollment.

Who is VITASIGNS?

We work closely with Paloma, providing clinical support staff, devices, and state-of-the-art technology. VITASIGNS is not your doctor, makes no medical decisions, and does not bill your insurance plan. Your care and your bill stay with Paloma.
